Under direct supervision the Wellness/Diabetes Program Manager is responsible for the development and management of the activities of the Yerington Paiute Tribe’s Direct Patient Care and Community based Diabetes/Wellness Programs. Supervises and is responsible for the planning, development, organization, and coordination of the YPTHC Diabetes/Wellness Program management, related budget and finance, quality improvement and safety management. Maintains confidentiality of all privileged information.

Duties

  • Collaborate with primary care providers to develop individual diabetic treatment plans and set patient goals that will work to promote a healthy life.
  • Promote physical fitness as a method to prevent and/or control diabetes for the Tribal community in collaboration with SNAP-ED Nutrition Educator.
  • Develop a schedule of physical and recreational activities for community members i.e., volleyball leagues, basketball leagues, sports clinics, fitness classes, fun runs, culturally relevant activities, etc.
  • Plan special events to promote awareness of diabetes prevention and control.
  • Develop community challenges to increase participation in physical activity. These challenges should provide incentives to all participants who achieve pre-determined goals.
  • Survey the community for input on the types of activities they want Wellness to provide.
  • Conduct Education campaigns through various media to raise awareness of activities to prevent or reduce the effects of diabetes.
  • Collaborate with Clinic Medical staff to refer participants to the Wellness Center.
  • Develop individual fitness programs for community members.
  • Maintain participation logs, use graphics to monitor participants’ activities and progress towards their personal goals.
  • Prepare reports for providers on members referred for fitness activities.
  • Collaborate with Education Department to develop activities for youth and families.
  • Maintain a safe and clean environment in the Wellness Center and Gym (equipment and facility).
  • Maintain the safety of participants in the Gym and Wellness Center.
  • Enforce established rules of conduct.
  • Attend training as assigned by supervisor to expand knowledge base and skill level, especially as related to the connection between physical activity and diabetes.
  • Provide education awareness to individuals and the community in areas of diabetic disease process, secondary body system complications, blood sugar maintenance, medication compliance, insulin delivery, foot care, ulcer care, making positive lifestyle changes, improving nutrition, and increasing physical activity.
  • Coordinate ongoing community diabetes education, community outreach, home base services and prevention activities in collaboration with the Nutrition
  • Education Coordinator and other Clinic/Tribal staff.
  • Conduct outreach to pre-diabetic community members to engage them in prevention activities.
  • Complete monthly report of program activities and outcomes for the Director.
  • Compile data and submit Annual Diabetic Audit to IHS.
  • Oversee Wellness Assistants and ensure that they are following the Daily Personal Training Schedule
  • Schedule Staff to Ensure the Gym and Wellness Center are Covered during Open Hours of Operations.
  • Perform other duties as assigned.

Qualifications

  • High School diploma or GED.
  • Associates Degree in Health Science Preferred
    2 years’ experience and/or education as a personal fitness trainer, sports coach, athletics, or other physical activity required.
  • Supervising Experience
  • Able to work evening until 9:00pm. Some weekends may be required.
  • Current CPR and First Aid certification required within 3 months of hire.
  • Be able to work independently with minimal supervision.
  • Excellent people skill and high regard for confidentiality.
  • Must have good time management skills and be able to complete tasks assigned in a timely manner.
  • Good writing and communication skills.
  • Basic computer skills preferred.
  • Possess a valid Nevada Driver’s License.
  • Pass Pre-Employment drug screen and complete background investigation.
  • Able to lift 40 lbs.
